NEAR MISS INCIDENT u A near miss incident n Results in neither injury or NEAR MISS INCIDENT u A near miss incident n Results in neither injury or property damage but has the potential to cause either or both n Occurs due to an unsafe act or condition n Requires corrective action to prevent recurrence l Examples: n n n 3/17/2018 A box sitting on an overhead shelf isn’t pushed back all the way when it suddenly falls and misses the employee standing below by a few feet Cords running across an area that people walk – trip hazard Icy sidewalks Wet / slippery floors Broken outlet covers 9

EMPLOYEE RESPONSIBILITY u Report ALL near miss incidents verbally to your supervisor or director EMPLOYEE RESPONSIBILITY u Report ALL near miss incidents verbally to your supervisor or director IMMEDIATELY n Unsafe Act l Immediately inform co-worker action is unsafe n Unsafe Condition l Remove yourself and others from immediate danger u Submit a “Near Miss Incident” Report n Immediately u Participate in the incident investigation 3/17/2018 10

EMPLOYEE RESPONSIBILITY u Report ALL accidents AND/OR injuries to your supervisor IMMEDIATELY n No EMPLOYEE RESPONSIBILITY u Report ALL accidents AND/OR injuries to your supervisor IMMEDIATELY n No matter how minor or insignificant they may seem u Report ALL accidents AND/OR injuries to the fiscal officer IMMEDIATELY n OSHA Regulations mandate the timeframe in which an employer must report an injury 3/17/2018 14

EMPLOYEE RESPONSIBILITY u Request medical attention if necessary n Return “Report of Work Ability” EMPLOYEE RESPONSIBILITY u Request medical attention if necessary n Return “Report of Work Ability” form to fiscal officer n Inform Supervisor of any work restriction n FOLLOW WORK RESTRICTIONS!!! u All follow-up appointments require a “Report of Work Ability” form to be filled out by doctor u Keep supervisor/ fiscal officer informed of on- going restrictions u Participate in the accident investigation 3/17/2018 15
