Who can register at Park Medical Practice ØThe practice accepts registrations from all Derby University Students, who live in the City of Derby as long as they can provide proof of course acceptance. ØUnfortunately we cannot register non Derby University students.
How to register to become a patient ØYou will need to complete our registration forms. ØThese can be found on our website www. parkmedical. org. uk ØOr pick one up from our reception Ø
Registration Forms These forms must be completed in full. If they are not completed fully it may delay your registration at the Surgery.

Change of Personal Details We may need to contact you urgently. Therefore, please ensure that your personal details are up to date in our records, including your mobile number, address and email address. If any of your personal details change, please tell us straightaway.
Text Messaging ØIf you supply us with a mobile telephone number we are able to send you appointment reminders and information. ØIf you do not want this service please advice us on you registration form or inform a member of staff.
The Summary Care Record ØThere is a new Central NHS Computer System called the Summary Care Record (SCR). The Summary Care Record is meant to help emergency doctors and nurses help you when you contact them when the surgery is closed. Initially, it will contain just your medications and allergies. ØPlease indicate on your registration form if you do not wish to have a Summary Care Record.

Sexual Health Services Ø A confidential Service for all Students at Derby University Ø If your worried you have a Sexually Transmitted Infection or have been in contact with someone who has make an appointment to see the doctor or nurse Ø Appointments usually available the same day

Cancelling an Appointment ØIf you have made an appointment and you no longer need it, please cancel it so we can offer it to another student. ØYou can do this by: ØTelephoning on 01332 747140 ØCancel online using your Park Medical Practice online login

A&E Accident and Emergency departments can be found at most hospitals. Ø They are there to help people who show signs of being very ill or are badly injured. Ø They should only be used in serious or life threatening situations. Ø o or life o o o o o Ø Serious medical emergency posing an immediate threat to a person’s health Head Injury – with loss of consciousness Overdose of drugs or accidental drinking of chemicals Severe allergic reaction Suspected Stroke Loss of blood Suspected heart attack or chest pain Severe breathing difficulties Severe burns Broken bones If you or someone else is having a heart attack or stroke, call 999 immediately.
Charges Ø If you are from the EU there are no charges for appointments at our surgery Ø If you are from outside the EU then charges may apply Ø You may need to pay for your prescriptions, £ 7. 65. You can apply for assistance with paying for these costs by completing a HC 1 form
What do you do if you fall ill when not in Derby? ØThe NHS allows patients to be treated as temporary residents at any GP practice throughout the UK. ØIf you go home (in the UK) and you need to see a doctor, you can be seen by registering as a temporary patient at another surgery. ØPlease do not register as a GMS patient outside of the Derby area if you intend to return to Derby University.
Patient Participation Group We are currently looking for people to represent the 18 years to 35 years category. The Patients who are on the PPG currently support the practice for a number of reasons including an interest in health related matters or in their local services, they may represent specific services users, or may want to put the volunteering on their CV. They may want to give something back to the practice or like the social aspect of the PPG. There are many more reasons why patients would chose to be in this group.
