Process and Procedures Step 1: INGOs submit a letter of request for stamp registration to COMINGO (via PACCOM), including detail information of the person authorized to do the procedure (full name, title, Do. B, passport number/ID number and a copy of Passport/ID). Final/Nov. 2013
Process and Procedure Step 2: COMINGO sends official letter to Police Department for Social Security Administration Ministry of Public Security, or Provincial Police Division requesting for the issuance of a stamp for an INGO (The letter should specify the information of the INGO including: full name, nationality, certificate of Registration (type, number, expiration) with a requested pattern of stamp (as sample drawing enclosed). Process and Procedures Step 3 The applicant comes directly to The Police Department for Social Security Administration - Ministry of Public Security or Police Division for Social Security Administration of province/city under the central government to submit the dossier, which includes: - An official letter of COMINGO mentioned above; - An original and a notarized copy of Certificate of Registration of INGO; - Personal Identification (ID)/original passport (as mentioned in the letter of COMINGO). Process and Procedures Step 4 The applicant comes to the Police Department for Social Security Administration Ministry of Public Security or Police Division Police for Social Security Administration of province/city under the central government to receive the stamp and Certificate of Stamp Registration as appointed date. Re-Issuance of the lost stamp The organization sends a letter of request for the reissuance of stamp, clarifying reasons, with approval of local police where the stamp lost, to the Police Department that issued the stamp and to COMINGO; Certificate of Stamp Registration issued by Police Department; A letter of COMINGO. Procedure for changing the Stamp If the current stamp is broken, out of shape, worn out…, INGOs should submit: - A letter of request for re-issuance to Police Department which issued the stamp and to COMINGO; - Certificate of Stamp Registration; - The old stamp which needs to change; - A letter from COMINGO. Procedure for Re-issuing Certificate of Stamp Registration If the Certificate of Stamp Registration is lost, torn… INGOs should submit: - A letter of Request for re-issuance of the Certificate of Stamp Registration to Police Department that issued the stamp and to COMINGO; - Current stamp (for checking); - A letter from COMINGO. Withdrawing the Stamp and Certificate of Stamp Registration Within 30 days since the regulation takes effect, the Head of INGO that is separated, merged, dismissed, or have accomplished its mission, transfer the form of possession, should submit the stamp and the Certificate of Stamp Registration to the Police Department that issued the stamp. If the stamp is temporarily suspended, COMINGO will send a request for withdrawing the stamp and announce to the Police Department and relevant agencies. Final/Nov. 2013
