Funded Graduate Students • Each semester students must be: • Enrolled in the Graduate School (professional students do not qualify) • Degree-seeking • In UK payroll with an assignment as a full-time TA, RA, GA (20 hrs. per week), or qualifying institutional fellowship recipient • Automatic enrollment • No cost to student An Equal Opportunity University

International Students • Insurance is mandatory for international students on F 1, J 1, or J 2 visas. • Automatically enrolled, cost is automatically applied to your student tuition bill • The charge for fall 2015 semester is $823. • The charge for spring/summer 2016 will be $1, 343. Funded international students • SHIP Office processes eligibility, the premium is paid by the Graduate School Funding Office (Judy Nave). • Allow one month for processing An Equal Opportunity University

International “waivers” • Outside plan from government sponsor or employer may apply to “waive, ” or cancel, SHIP online via uky. myahpcare. com. • Waiver application deadlines apply. – 9/22/15 for fall – 2/22/16 for spring/summer An Equal Opportunity University
