Paperwork School: Risk assessments Children: There are two essential pieces of paperwork that need to completed as soon as possible and handed in over the next week please- Deadline Friday 7 th Oct 1) General information sheet. 2) Emergency contact & Medical Info sheet
First Aid Medication Please provide it in original container with full instruction for use and labelled with your child’s name. Also complete the medical form and return to the office as soon as possible – Deadline Friday 7 th October This needs to be signed by Mr Smith before we can take it. Medication to be handed in on the day of the trip to Mrs Harding- first aider allocated to looking after, and administering, the medication while we’re away. Is the medication in date? Unexpected first aid issues.
