(Vision benefits will continue to be on a 24 month basis. The 24 months begins the month you have your eye exam, or purchase frames, lens, or contact lenses. ) – This year only, because of the elimination of 4 plans, 4 th quarter deductible carryover will apply for all plan changes. 19
