• SHS work Immersion – no immersion after 5 pm (7 am to 4 pm or 8 am to 5 pm) MOA should be explained to the parents and students, Medical Certificate is a requirement (Through Division Medical Unit). The learner are in the immersion place to observe what is going on, they can do only the basic/soft skills, so there should always be RELECTION every day. • Implementation of Project 555

Dep. Ed Order No. 3 S. 2018 • Basic Education Enrolment Policy – Learners aged 13 and above who shall be enrolling in basic education for the first time shall be advised to undergo ALS Program – Grade 7 – PEPT Passer, A and E Passer and PSA Birth Certificate – Grade 10 – SF 9 Grade 10 Formerly form 138 – Grade 11 PEPT Passer, A&E Passer and PSA Birth Certificate

• In the absence of PSA Birth Certificate, the parent must submit – BC form local Civil Register or Brgy Certification containing basic info of the child (submission must be done once during the duration of a child’s basic education. ) – If the documents mentioned above are not submitted by the end of Early Registration, learners will be given until August 31 st of the school year they will enroll in to produce and submit the required documents.

– Requirement for Transferees • Learners from public or private schools shall submit their SF 9 (formerly form 137) or a letter certifying the last grade level the learner completed signed by the school Registrar. (DO no. 54 s 2016) • Special cases- Phil School Abroad-Birth Certificate and Permanent Transcript of records • Foreign School Abroad-Latest report card. Accepting school shall have the discretion to determine the appropriate grade level in which to place the learner (DO 2 s 1994)

• Learners from non-deped-PEPT/PVT Certification of rating, Birth Certificate from PSA or Local civil registrar, or brgy certification • A&E Passer-A&E elementary level certification of rating, birth certificate from PSA or local civil registrar or brgy certification. • Learners who transfer to ALS shall not be considered school leavers. The Adviser will make the necessary notations on the learners’ school form 1, signifying that the learner transferred to the ALS Program.

• Fees – No fees shall be collected from school children during enrolment – Non-payment of voluntary contributions shall not prohibit enrolment of learners – Collection of authorized voluntary contribution should not be done during the release of financial assistance for conditional cash transfer or 4 Ps recipients. • Enrollment data management – Every learner –only one unique and permanent LRN • Effectivity – SY 2018 -2019 onwards

– Any erasure or alteration should be validated and bear the signature of the school head in ink – It shall be printed using ordinary legal size bond paper. Commercial printing that will result in unnecessary expenses is hereby discouraged. – Teachers, school head and all concerned Dep. Ed personnel are cautioned against individuals and groups that may attempts to sell electronic or print copies of these new forms.
