Overview of the Elevator Division and Its Quality Assurance Program January 2006
WELCOME! Mission Our mission is to ensure the safe and lawful use of elevators, escalators, amusement rides and other related devices by enforcing codes and standards. We are also charged with facilitating development. Safety • Riding public • Maintenance personnel • Inspection personnel We enforce • ASME A 17. 1/ 1996 -97 • NYC Building code We support Development by permitting new technologies under pilot programs.
Overview: • 62, 000 devices in NYC- Elevators, Escalators, Amusement Rides, Man lifts, Conveyors, Wheelchair Lifts, Personal Hoists devices • 12% of the elevators in the United States are in NYC • The average elevator makes approximately 500 trips a day, which is total of approximately 30 million trips per day • Elevators and escalators in the City of New York transport more riders than the MTA subway system • Elevators are one of the safest modes of transportation - the chance of an accident is 1 in 12 million

When do you need to file an application? 1) New Installation 2) Alteration (change in speed, capacity, rise, structural and location) 3) Replacement and modification ( replacement/ modification of controller, machine, governor, safety etc. ) 4) Removal 5) Dismantle 6) Self-Certification for minor work (no testing required by the code) Note: Acceptance test is required for item # 1 to 5 types of applications and NYC Buildings inspectors shall perform the necessary test and inspection.
Types of Inspections • Quality assurance inspection • Acceptance Tests - Permitted Application – Perform 500 Inspections per month – $200 Penalties for Unprepared and No Show – Expedited Inspections for handicap, elderly, and high rise • Complaint Inspections ( Received through NYC 311 call center) – Perform 1000 -1200 inspections per month (A and B types of complaints) – All complaints response times are within mayor’s office target • Accident and Emergency Response (24/7) – 110 - 150 accidents per year. Majority of the accidents are minor – Department conducts penetrating inspections in all cases of injury and death. • Survey Inspections - Confirmation of the device(s) at the location. • Routine Inspections - Accomplished primarily by contract inspectors • Violation Re-inspections • Amusement ride inspections
Amusement Rides • Perform an average of 1200 - 1500 ride inspections per year • Currently there approximately 175 permanent rides in NYC • Approximately 900 - 1200 Temporary Ride (Carnivals and Shows) inspections are conducted each year • Amusement ride inspections are joint ventures with Department of Consumer Affairs and Buildings. Training • All our inspectors have attended the Qualified Elevator Inspectors (QEI) training program. • All Supervisors and Chiefs regularly attend leadership training classes. • All amusement ride inspectors have attended the National Association of Amusement Ride Safety Organization (NAARSO) training program. • Seminars on new technology provided by outside speakers. • All newly hired inspectors are mentored by selected senior inspectors. • PDA Training - All Elevator Inspectors are currently using PDAs as a referencing tool for code – The PDA eliminate the needs to carry around different code books – The PDA also improves efficiency and saves time of inspections • In house computer and management training provided by department training center. • All inspectors attend weekly in-house training classes.

Quality Assurance Inspections of Staff • Supervisors conduct two Quality Assurance inspections a week • Chiefs conduct three Quality Assurance inspections a month • Deputy Directors conduct two Quality Assurance inspections a month • Director conducts one Quality Assurance inspection a month • Every inspector receives a quality assurance visit twice a month • Database is develop as a tracking mechanism for quality assurance inspections • Elevator Division has published an Operational Manual to ensure uniform standards within the Division
Quality Assurance Inspections of Contractors (Periodic Inspections) • Contract awarded for monitoring contract inspectors. Contractor will ensure the quality of inspections performed by other contracted agencies.
Quality Assurance Inspections Licensees (Self_Certification) • This is a newly created area of focus, designed to promote quality assurance of self certification and elevator tests conducted by the elevator companies • We perform the following Quality Assurance Inspections: –Five years tests on Elevators –Self Certification of Violations issued by contract inspectors –Self Certification of Applications –Two years tests on Escalators

Type of Enforcement • Environmental Control Board (ECB) Violations – Failure to maintain – Work without permit – Failure to maintain - Hazardous 1 - First Offense 2 - Second Offense • Department of Building (DOB) Violations • Criminal Court Summons • PVT Violations by the contracted inspection company
Outreach Program • Elevator Advisory Council • Amusement Rides Advisory Council • Our advisory councils publish technical bulletins based on lessons learned during our inspections, testing and investigations to enhance the safety standards and codes enforcement • Elevator and Escalator Safety Week Program in city schools – On the 3 rd week of each November our staff visits 6 to 8 schools to provide safety awareness classes to the 2 nd grade students – This program is a joint ventures with the MTA (Metropolitan transit authority), Elevator Conference of New York, Department of Education and National Safety Foundation
