What is Care Management? MIDAS Care Management system is the physical MIDAS database that collects data: • Interfaces with Meditech Admissions/ MIS • Interface with Meditech Discharge Abstract • User Input to the MIDAS Care Management Subsystems • User Input to MIDAS Focus Studies The data in all modules is integrated for use in reporting, alerting and benchmarking
What are the main sub-systems? MIDAS Care Management has several subsystems that house data input by users or interfaces: Examples: • Hospital Case Management • Patient Reviews • Patient discharge and placement • Quality Management • Quality Event Reviews • Case and Peer Reviews • Infection Control • Risk Management
MIDAS Navigation Function Menu Registration Inquiry Encounter Emergency Room Inquiry Observation Inquiry Encounter Inquiry Discharge Inquiry Patient: Catherine Tatro MRN: 0000268373 Acct #: 60979226
What is a worklist? Worklists give you a list of all patients requiring review who have met specific, user-defined criteria. Worklists are assigned to a user in the system. You may pull up other users worklists Examples: • CM – AMI Admit CC • QM Mortality • Left AMA or WO Being Seen Patient: Catherine Tatro MRN: 0000268373 Acct #: 60979226
What is a focus study? A Focus study is a data collection tool that can collect data in the Care Management system or through internet based data entry. It may be at system, patient or encounter level. Some are pre-determined such as Core Measures but may also be setup based on our needs Examples: • CM – AMI • QM Mortality • CCU Goal Sheet Patient: Catherine Tatro MRN: 0000268373 Acct #: 60979226
Quality Management vs. Review? A Quality Management entry is the first level review of any quality event, this is the case write up or first step in a case being referred to Case Review, Peer Reviewor other internal QA process. A Quality Review entry is initiated if the initial entry is found to warrant further review than just trending. Quality Review examples: PSI’s, HAC’s, Unanticipated outcomes such as complications found in data vision. Quality Review Examples: Case Reviews, Peer Reviews, Departmental M&M Review
Indicators: Graphs vs Profiles? Indicator Graphs: Individual indicators, usually comprised of a numerator and a denominator that display a rate in graph or SPC chart Example: Total Inpatient Deaths Indicator Profile: Group of indicators within a clinical topic, shows an overview of all measures n, d, rate and bundle “all-or nothing” measures Example: Mass. Health P 4 P, AHRQ Patient Safety Indicators, Core Measures AMI
Reports: Standard and Repor. Track Standard Reports: System built reports covering a variety of system analysis and general reporting. Some reports contain Macros for more detailed analysis. Examples: Datavision Readmission Report. Track Reports: Custom built reports that can be output to printer, excel, HTML (webpage view), email attachment Quality Reports prefixed with QM Example: QM Mortalities List
