M-ton’s “fantastic view” ferris wheel: the automated patient history 12/13/07
Would you like. . . • having half of your visit note done before you even walk into the patient room? • having a detailed summary of your patient’s concerns before you even walk into the patient room? • having your PCO note done when the patient leaves?
Instant Medical History http: //www. medicalhistory. com/usertools/videos. asp (FFM project)
how it works • patients answer a series of questions • program prompts for chief complaint -> appropriate follow-up questions • lay language -> clinical language • automatically populates your EHR note
what the patient does. . .
. . . what you get!
http: //www. medicalhistory. com/usertools/videos. asp (interfacing with amazing charts)
Patients like it & can do it! • most patients like using the software • most patients can use the software (and if they can’t, you now know about another problem. . . )
Why not use paper? • program can flexibly react to answers (e. g. , positive 2 question depression screen -> PHQ-9) • you don’t need someone to enter the data • you can add screening questions relevant to your own population • patients more likely to enter sensitive information into a computer than tell you
weaknesses • computer cannot sense nonverbals • computer must be maintained • computers require space • some patients don’t like it • doctor can end up with too much data
benefits • patient better prepared for interview • doctor edits data instead of entering it (much faster!) • language translation available • research and QI database easily available • more documentation = more $$$
too futuristic? • already in use at and • software is available that can interact with • can you imagine this software in our own FHCs?
