Health Insurance • Includes $5, 000 term life insurance policy • Family plan covers: – Spouse – Former spouse – Children to age 19; handicapped over 19 – Students age 19 -24 (must prove student status) • Effective Date: First of month after 60 day waiting period • COBRA – you may continue coverage from previous employment during the waiting period • “B” Waiver – if you incur expenses you can buy back coverage after the fact National distinction in health sciences education, research and public service

Long-Term Disability – Guardian Plan • PROVIDED AT NO COST TO ALL RESIDENTS AND FELLOWS • Covers disability which is not work-related • Waiting period for benefit - disabled for 180 days • Effective Date: Upon date of hire • Pre-existing Condition: No coverage unless 12 months treatment free or 24 months of paid premiums • Benefit Payments: 66% of salary up to 10, 000 per month / $100 monthly minimum (TAXABLE INCOME) • Portable – Can continue up to $3, 000/month of coverage when training ends without medical underwriting; • Supplemental coverage options during training
Massachusetts State Board of Retirement • Mandatory participation -9% of pay to $30, 000; additional 2% above $30, 000 -Pre-tax • In lieu of social Security • If leaving State Employment prior to retirement, this is fully refundable. Paid Time Off • Holidays – Residents and fellows may be expected to work on holidays. They do not receive comp time or pay. Programs establish individual policies concerning alternate time off or assigning holiday time. • Personal Time – 3 days awared upon hire (pro-rated for part-time or late (after July 1) hires • Vacation Time – Three weeks (pro-rated for part-time) awarded upon hire. – Must be used during the academic year. Sick Leave Bank • Donate 16 hours (two days) • Must be a member for 6 months prior to using the bank • Must be on approved FMLA • Must use all accrued time first • 480 hours maximum (12 weeks) • Requires Committee approval This provides the bridge until disability insurance kicks in.
