PROGRESS -DATA • Analysis of Wide Range of Reports and Surveys • Predominant Error PROGRESS -DATA • Analysis of Wide Range of Reports and Surveys • Predominant Error Types • Installation Errors • Incorrect Installation • Incorrect Maintenance • Dominated by knowledge-based and rule-based error • Common Performance Shaping Factors • Pressure • Training • Fatigue • Value of Human Factors Programmes • Conclusions? Work in progress, but …. Design …. . Error Prevention …. Error Recovery

We have come a long way, but challenges remain l Regulators focused on compliance We have come a long way, but challenges remain l Regulators focused on compliance rather than effective implementation l Regulators are not looking at the intent of the regulation l Implementation has been very training centric l And that training has focused on cost/compliance …and these organisations are not reaping the benefits and are becoming increasingly frustrated!

Is this an organisation you know? ü HF? We've done that… ü We published Is this an organisation you know? ü HF? We've done that… ü We published our HF procedures ü We trained everyone in HF ü We comply with all current regulations. ü Our regulator audited us and it didn’t result in any findings regarding our HF programme ü We fix problems as they occur

What does a healthy HFEM programme look like? What are the Key Performance Indicators? What does a healthy HFEM programme look like? What are the Key Performance Indicators? l A healthy reporting culture indicates: l l Senior Management buy in l A real Just culture l Effective error investigation l Feedback system is working l Effective interventions l A reduction in error costs l If its monitored!

THE MESSAGE l We are heading in the right direction l It’s about changing THE MESSAGE l We are heading in the right direction l It’s about changing cultures and behaviours l It’s about effectiveness not just compliance l Our success is measurable ……its going to be a long journey ……. …. . but it is a shared journey
