Why such a program? w Diabetes is common in HK w Multiple complications require multiple specialties w We need a network to link up the GP, specialists, allied health, tertiary hospitals and the industry
Aim of the program w Enhance the co-operation between the primary care and secondary care doctors w Enhance the co-operation between the private doctors and the tertiary care hospital(s) w Improve the standard of diabetic care w Reduce the incidence of complications
Target group w GP and specialists with interest in diabetic management w Clinic staff to help the doctors
Highlights of the program w A certificate course for GP in the area w A course for the clinic assistants of private clinics w Publish handbook (GP, specialist , allied services) w Internet discussion group w Resources Centre w Patient support group
Certificate Course for GP w Aim: update knowledge on DM w Improve treatment skills w Improve team approach w Certificate to indicate interest of the doctor in DM
Course for clinic assistants w Aim: train the clinic staff to help the doctor w Contents – – Basic concepts on DM BMI, waist measurement BP and blood sugar monitor Insulin injection and training
Program Handbook w List of GP and specialists with interest in DM management in the area w List of allied services w Indications for referral to specialist and tertiary hospital(s) w Protocol for referral to HA hospitals w Advertisements
Internet discussion group w Discussion group on diabetic management w Diabetic news w Special offer from the industry

Resources Centre w Collection of publications and other resources w Resources from Diabetes HK w Reference centre for microalbumin
Reference Centre for urine microalbumin w One of the services to members of the network w Members can send urine for microalbumin test for diabetic nephropathy w Urine test for microalbumin with Microtek w If positive, confirm with DCA 2000+
Future direction w Agree on Treatment protocol w Encourage patients to attend doctors in the network group
