GA 2 – Member School Directory Update • For changes in school’s contact info or an administrator or coach. • Letters or e-mails from the Principal saying there is a new AD and/or FHSAA Representative are not official. • If there is a change in FHSAA Representative, then only the Principal can sign the form. • Changes on this form will only affect the FHSAA internal database. It does not change the C 2 CSchools. com database. The Principal, AD, FHSAA Rep and athletic secretary all have access in C 2 C to add/delete/modify logins. Insurance Guidelines • The FHSAA is supposed to be listed as an “Additional Insured” or “Certificate Holder. ” • If your school changes providers or buys a new policy, please add the FHSAA as an “Additional Insured” or “Certificate Holder. ” • Normally general liability policies are on separate certificates from the catastrophic and student accident policies, and can also be covered by different insurance providers. • Business Managers can sometimes get upset when the Association asks the FHSAA Representative at the school for the insurance certificates. Insurance Guidelines continued • Many schools have policies that renew in the summer, especially in the month of August. • If a school is looking to change providers, there are three companies that handle the largest number of schools within the Association. • Update fax information. With the FHSAA’s e-fax system, every administrator has his/her own dedicated e-fax number. The one for insurance is (352) 244 -5050 and the contact is Seth Polansky. Sports Participation Surveys • Required submission for all member schools. • Do not confuse girls participants on a boys team with a girls team. • Not the same thing as district Title IX reports! F L O R I D A H I G H S C H O O L A T H L E T I C A S S O C I A T I O N 7
Member School Population Report • Use FTE Week numbers in October for public schools. Private schools do not report FTE numbers and can simply report student enrollment at the time when the FHSAA’s electronic form ready. • Please make sure numbers and totals are accurate! Member School Personnel List/ FHSAA ID Card Order Form (GA 1) • Due by September 9. • List all coaches and other key athletic personnel with home/cell phone numbers and e-mail addresses. • Do not type “same as above” or simply putting ditto marks (“”). Miscellaneous Items • C 2 C Schools: Most of the FHSAA’s forms are already found on the C 2 C Schools site in the “FHSAA Library” section. • FHSAA Broadcast e-mails: These go out to ALL schools, even if the message is intended for some. If you are not getting them for a period of a few weeks contact the FHSAA. • Member schools that have middle school programs are allowed to play non-member middle schools as per Bylaw 8. 3. 2: – 8. 3. 2 Middle, Junior, and Combination Schools. Member middle, junior, and combination schools may engage in interscholastic contests with nonmember middle and junior high schools provided that: o (a) Competition is confined to middle and junior high schools in the same county under conditions established by the District School Board for that county; or o (b) Subject to Executive Director’s approval, the nonmember school is within a 50 -mile radius of the member school. FHSAA/Fred E. Rozelle Sportsmanship Awards • Awarded to schools that best exemplify good sportsmanship during the school year. • Up to 28 section winners and seven overall winners. • Section winners receive $500 and a plaque; overall winners receive an additional $2, 500 and a larger plaque. • Nomination forms available in March. • Self-Evaluations due mid-April. • Selection Committee meeting/notification of winners in mid-May. • Awards are presented at school events in the fall. Sportsmanship Awards cont. The selection committee considers: • Quality of the nomination, which must be submitted by another member school, officials’ association or county AD. • Self-evaluation form submitted by the nominated school, including supporting materials. • Exemplary sportsmanship reports filed with the FHSAA Office throughout the year. Florida High School Athletic Hall of Fame • Nominations for Class of 2011 due November 1. • Nomination form available in PDF format under general forms on FHSAA. org. • Include detailed biographical information and support materials. • Demonstrate nominee’s impact at the state level. Academic All-State Team • 12 male and 12 female recipients from around the state. • Schools may nominate one male and one female graduating senior with a minimum 3. 5 unweighted GPA and two varsity letters in two different sports each of their junior and senior years. • Team members receive $1, 000 and a medallion. • Male and Female Scholar-Athletes of the Year receive additional $3, 000 each and a trophy. Lay Sunshine Cup All-Sports Awards • Schools receive points for state series appearances throughout the year. • Bonus points for district and regional championships. • Points deducted for unsportsmanlike behavior reports. • Awards given at the end of the year for Girls, Boys and Overall in each classification. • Awards are presented at schools in the fall. This request must be made by the school’s athletic director using the online system; o 39. 4. 6. 1 If a school is unable to secure a credential for a student, an adult staff member of the school or an adult at an outside company who has been hired by the school and had a background check performed may be issued a credential. FHSAA State Series Broadcasting • All radio broadcasts, telecasts, cablecasts and webcasts of FHSAA State Series events must be approved by the FHSAA Office. • Form AT 11, “Agreement for Media Rights to FHSAA State Series Events, ” is available on the Media Services page on FHSAA. org; deadline is 3 p. m. the day before the contest. • The host school will receive a fax of the agreement from the FHSAA Office.
