Referral form received via email in PDF format: BMI/ smoking status/ Age/ BP. (Size population known). Criteria for referral: > 16 years of age Sedentary lifestyle HTN/ DM/ Cholesterol/ Stable angina/ arthritis/ asthma/respiratory problems/ obesity or overweight / osteoporosis/ depression/ anxiety/ MS/ PVD/ CHD If CHD - Additional form needs to be filled out Online referral system generates a letter, which is sent to patient asking them to contact ERS office Patient phones to book an appointment with ERS office No phone call – drop out Screening appointment/ Assessment at centre: Screening data, PAR-Q, RFA, BMI, AP, AGE. Ethnicity, Occupation, SF 12 SUITABLE 1 year membership to scheme UNSUITABLE Scheme does not meet clients requirements.
