H) should continue to be engaged to facilitate training or relevant professionals from the field Three days for the in-country training. Still due to insufficient knowledge of English it can be challenging to have the Distance training passed The inter-agency country team approach should be maintained and further provided with updated materials. Title: Reproductive health in Crisis (UNFPA)(2015) n n n n Recommendations Monitoring and evaluation (M&E) is needed to ensure the quality and effectiveness of the training and subsequent multiplier trainings The length and focus of the training should be flexible taking into account the specific context and the participants’ background. Key documents should be translated into local language(s). Specialists from other Cos can be invited to assist in facilitating challenging subjects in the training A mechanism to follow up with and keep track of trainees should be established The country team should meet with the Mo. H and the national disaster management/planning agency before future training to support understanding and buy-in A funding source for provincial roll out of training or action plans should be secured The in-country training should be adapted to the national context of each setting, in terms of content, structure, and length The training structure needs to be adapted for less technical participants. H platform for risk managing, legislation, orders of minister etc. Infrastructure of some health institutions is not in satisfactory conditions. . a possible civil emergency (snow, flood, etc. ), the heath care staff has more difficulties to provide quality health care assistance to the pregnant woman, child, under normal circumstances much more in emergency conditions not only because they lack many medical devices, drugs, etc; 29
Title: Reproductive health in Crisis (UNFPA)(2015) n The supply with emergency medications, according to the base of the primary service package was incomplete, insufficient, in some area lacking. n Lack of human resources (doctors and midwife-nurse), n Breast feeding was a problem, although it is a condition for reducing morbidity and mortality, particularly in emergencies. n Cold chain for vaccines functions normally etc. 30
Title: Reproductive health in Crisis (UNFPA)(2015) n Some Recommendations: MISP to be included in particular in Mof. Project component A: Monitoring and Evaluation of EPC Training in the Maternity Hospitals of Durres and Kukes Monitoring and evaluation were carry out by using the “Assessment tool for the quality of hospital care for mothers and newborn babies” The aim of the Qo. C assessment is to help Ministry of Health, and hospital managers and relevant staff at facility level, to identify areas of pregnancy, childbirth and newborn care that need to be improved, to discuss priorities, and to develop plans of action to implement changes 32
Title: Improve Quality of Care for maternal and child health and Implement BTN initiative (UNFPA) (2015) n n n The specific objectives: The objectives of follow-up, monitoring and evaluating after EPC training courses are to acknowledge achievements. Findings of the reassessment There have been variable, but in some cases substantial improvements, in key aspects of case management (care during normal labour and delivery, and care for sick newborn babies). . The areas related to care for newborn babies show a slightly improvement. There was a moderate improvement in management of obstetric complications and infection prevention. Overall, there has been substantial and generalized improvement in the general environment and friendly care for both mothers and babies. Areas that showed improvement were: monitoring and follow up, initial development of protocols. Statistics are collected regularly but record keeping is substandard and need to be updating. A unified medical record for delivery care is still not available; The system for prenatal referral and regionalization is existing in principle. Continuum of care throughout pregnancy, childbirth and postpartum period for mothers and children, within different levels. 33
Title: Improve Quality of Care for maternal and child health and Implement BTN initiative (UNFPA) ( 2015) 34
Title: Improvement of health care quality for mothers and newborn babies in maternity hospitals in Albania (UNFPA) (2015) n The purpose of this report is to present the final set of project activities, findings, conclusions and recommendations for further improvements of Quality of Care for Mother and Baby that are a direct result of a UNFPA financed project that (i) Monitored and Evaluated the measureable outcomes of EPC Training in the Maternity Hospitals of Durres, Kukes and Shkodra and (ii) Carried out a Beyond the Numbers initiative that reviewed incidents of near miss cases in the respective hospitals to look for improvements in practices to reduce risk factors. n Activities: n The Monitoring and Evaluation of the measureable effects of EPC Training in Three Maternity Hospitals in Albania The applied initiative: Beyond the Numbers – Utilizing a different way of analyzing the mortality and morbidity of mothers and newborn babies from an analysis of near miss cases Final Round Table Meeting in Tirana n n n 35
Title: Improvement of health care quality for mothers and newborn babies in maternity hospitals in Albania (UNFPA) (2015) n Findings and Conclusions of the Beyond the Numbers Study n Record keeping in general requires more detailed information in order to facilitate informed opinions of solutions to reduce the risks in cases of ‘near misses’ in order to avoid or reduce such occurrences in the future Details of PPH require more measured values in terms of vital signs, volumes and time elapsed. There was found to be insufficient data that could be called quality data; the capacity to analyze and interpret statistical data and use them as a guide for action is underdeveloped for near miss cases n n Preparedness of patients at risk, for instance in cases of known anaemia, is an area for improvement n General preparedness and awareness of all clinical staff to be ready to deal with near miss incidents is an area that needs developing as a core subject in Continuing Medical Education training modules n There are examples where the physical institutional problems e. g. laboratories not readily available on the same site that create additional risk factors. In such cases pre-planning for emergencies must be emphasized to the administration 36
Title: Improvement of health care quality for mothers and newborn babies in maternity hospitals in Albania (UNFPA) (2015) n A general lack of the use of local protocols is a continuing weakness n In one of the study cases the lack of a viable system for perinatal referral was again identified. There is a need for guidelines to fix the criteria on which referrals should be based and to ensure that there is the logistical support of suitable transport for all weather emergencies. This again is mentioned under this component also a serious inadequacy 37
