Individual Candidate Responsibilities u u u The reappointment and/or appointment candidate will be responsible for writing the first draft of the education officer report. The reappointment and/or appointment candidate will be responsible for documenting his or her teaching contributions every year, including collecting of evaluation data if not included in HAMSTER report. EDUCATIONAL DATABASE Section 2 Please provide a personal statement from the candidate outlining his/her teaching focus, strengths and accomplishments. It should include significant contributions such as innovative teaching materials created by the candidate. The statement can be no longer than ONE page. 5
EDUCATIONAL DATABASE Section 3 List all teaching activities in which the candidate has participated during the past three years. The activities should be organized by year and by type. For each course or other teaching activity, provide details about the frequency of teaching and the hours of contact with student (s). The template for section 3 will contain discreet subsections for undergraduate medical education didactic teaching, clinical teaching, mentoring and/or research training, and continuing medical education. EDUCATIONAL DATABASE Section 5 A Please provide names, addresses, and e-mail of up to 10 individuals (medical students, graduate students, house-staff, physicians in continuing medical education, nurses, other health care professionals, etc. ) whom the candidate has taught, particularly during the past 3 years, indicating whether contact was on an individual basis or in a small group setting. The Subcommittee on Teaching Evaluation will solicit letters from all these individuals. NOTE: Once five letters have been collected, the dossier will be prepared for review. EDUCATIONAL DATABASE Section 5 B Please provide evaluation data of the candidate's teaching performance, including quantitative ratings. Evaluation data for undergraduate medical education and graduate medical education teaching will be found in the individual candidate’s HAMSTER report. It is up to the candidate to collect and maintain evaluation data for BGS and CME activities. This data should be referred to in the Education Officer report. 9
EDUCATIONAL DATABASE Section 6 A Please provide the education officer report which evaluates the candidate's teaching performance. This report should provide a detailed summary of the faculty teaching activities, including the degree of participation and a performance evaluation for each activity. As noted earlier, this report should also refer to the evaluation data from Section 5 B. Please note – if there are outlier data in Section 5 B, it must be explained in this report. 10
EDUCATIONAL DATABASE Section 6 B If applicable, please provide a letter from the chair of the graduate group with which the candidate is primarily affiliated, describing the candidate's participation in graduate student training. This primarily applies to basic scientists. Section 6 C Other peers. If appropriate, evaluations of teaching performance maybe submitted from other peers who have had an opportunity to observe the candidate teach. The Subcommittee will not solicit these letters, they will need to be provided by the candidate. 11
