Odyssey House Martindale Clinic Goals • • To provide a medical home for the behavioral health population Provide evidence based integrated care that improves patient outcomes • Reduction in incidents of chronic diseases (diabetes, etc. ) • Reduction in relapse • Reduction in recidivism • Reduction in lengths of stay • Reduction in mental illness symptoms • Reduction in psycho-somatic symptoms
Odyssey House Martindale Clinic Patient Overview • • • Present with significant deferred medical conditions Chronic diseases as a result of their behavioral health condition (diabetes, heart problems, etc. ) Have minimal history accessing medical care • Typically use the ED as their primary care provider • Have limited or no access to medications to treat medical or behavioral conditions
Odyssey House Martindale Clinical Overview • • New patient physicals Acute care – Sprains, Infections, etc. Chronic Care – High blood pressure, High Cholesterol, Diabetes, etc. Psychiatric Evaluation and Medication Management Referral to behavioral health programming Wellness – Smoking Cessation, Physical Activity Education on appropriate accessing medical care
