CFETP/AFJQS Documentation. All personnel authorized to sign off tasks in Part II of the CFETP and 797 must be listed on the Identification Block of the CFETP, Part II. Ensure the trainers and certifiers are listed on the current Flight Trainer/Certifier Listing.
This Block needs to be filled in as Last Name, First Name, Middle Initial format.

Transcribing documentation to a new CFETP is an administrative function, not a re-evaluation of training. Use the new CFETP to identify and certify current training requirements and to retain previous qualifications from the previous version. Maintenance of CFETPs for personnel in retraining status. Maintain CFETP from previous AFSC until commensurate skill level is achieved, then give the obsolete field CFETP to the individual. Decertification and Recertification. When a supervisor determines an airman is unqualified on a task previously certified for their duty position, the supervisor erases the previous certification, or deletes certification when using automated system. Appropriate remarks pertaining to the reason for desertification are entered on the AF Form 623 a. Begin recertification (if required) following procedures in documentation paragraph above.

AF Form 797, Job Qualification Standard Continuation/Command JQS. The AF Form 797 is a continuation of the CFETP, Part II, or AFJQS. It defines locally assigned duty position requirements not included in the CFETP, Part II. The AF Form 797 disposition is the same as the CFETP and AFJQS. NOTE: Career knowledge items require an evaluation tool to verify attainment of knowledge.

Ensure that this block is filled in Ammo, Joe P. AF Form 623 a
AF Form 623 a, On-the-Job Training Record Continuation Sheet. The AF Form 623 a will be used to document an individual’s training progression. This form will be used to reflect status, counseling, and breaks in training. Just about anything may be documented on the 623 a as long as it pertains to the members’ training. Examples include initial CDC issue, CDC completion schedule, explanations of delays in CDC completion or training requirements, problems encountered with task certification (if any), and any training related counseling statements. When used for training-related counseling, include strengths, weaknesses, areas to improve, and means to improve. Both the supervisor and the trainee must sign and date any entry. Maintain the AF Form 623 a as long as it pertains to the current training objective (i. e. , award of the skill level or completion of qualification training). The supervisor will determine if any additional AF Form 623 a will remain in the training record.
AF Form 623 a, On-the-Job Training Record Continuation Sheet. AFI 36 -2201 states that the supervisor will make notations on the 623 a “as required” which is an ambiguous term. Generally, at a minimum, supervisors should periodically: 1. Check the upgrade training status of the trainees appointed to them. 2. Check the trainees’ OJT records for errors or training opportunities and/or shortfalls. 3. As required, make a notation on the trainees’ 623 a giving training status of OJT, CDC, and Ancillary training. A lack of notations on the 623 a for long periods would give the appearance of a lack of training whether one existed or not. Remember, the ultimate responsibility for the members’ 623 OJT training record falls upon THE SUPERVISOR. Although, the trainee has a vested interest in his/her own training records and should remain responsible for their own records. Supervisors take a minute to read this page again
