Differences. . • • Higher cholesterol after age 50 Higher CRP (x 2 -50 more autoimmune diseases) High TGs is a more potent risk factor DM women - much higher CAD mortality c/w DM men • Less mortality reduction in DM women • Clustering of post menopausal risk factors: obesisty, HTN, lipids… (hormones? )

Differences. . • Framingham risk score underestimates risk in women – classifies >90% as low risk !! • Reynolds risk score – sex specific tool • c/w FRS – the Reynolds risk score – classifies > 40% of women as intermediate risk

Women: more complaints less CAD • More subjective angina with less obstructive CAD – Age < 65 y c/w men: 27% vs. 11% – Age > 75 y c/w men: similar rates of angina • > 50% of women with no CAD have c/o angina • BUT: women with non-specific chest pain have x 2 MI and more mortality from CAD • ASA less effective in women for MI primary prevention and more Stroke primary prevention (JAMA 2006) • • • Women with ACS: more NL CAs In ACS pts - women had 50% less CAD STEMI with NO obstructive CAD: – Women: 10 -25% (Takotzubo 80 -100% are women) – Men: 6 -10%

Women: less disease mortality • Cose older age at presentation ? – Sex differences in younger women too. . • More comorbidities… – cose they r older at presentation? • Less adherence to guidelines therapies. . – cose more complaints and less objective disease at CATH and SPECT ? ?

ETT - differences • 1 mm ST depression poor sensitivity in women: 65% • 2 mm ST depression better marker of ischemia in women • Women often can not reach 5 METS on the trademeal > increased risk x 3 for IHD mortality and MI • < 7 METS poor prognosis in obese and lean women

SPECT: differences • Minor perfusion abnormalities undetected in small hearts • Breast artifacts (attenuation correction algorithms diminishes frequency of artifacts) • In ambulatory population (34, 000 M & 22, 000 F) women with ischemia: – Age 55 -75 x 2 mortality – Age 45 -55 x 12 mortality

Microvascular reactivity • SCD autopsies in women: more CA erosions and microembolization • Retinal artery narrowing correlates with CAD in women but not in men • Smaller arteries and more prominent positive arterial remodeling • … may explain the higher rate of ACS and less severe CAD of large epicardial arteries ?

SCD - differences • SCD at younger age in men (hormonal protection? ) Men: M~ age 65, Women: M~ age 71 • CAD less common in women with SCD Men: 49% Women: 40% • More unexplained SCD in women • More women with SCD have good LV fx. Women: 49% Men: 35% • More men with SCD have severe LV dysfx. Men: 35% Women: 21%
